PARTICIPANT GUIDELINES & RELEASE OF LIABILITY 

AGREEMENT TO FOLLOW POSTED GUIDELINES. I acknowledge that Pickleball Euphoria is operated inside a leased church property. While onsite I will be respectful of all parties on the premises. I will refrain from using vulgar language or playing music with vulgar language or references. I will not throw paddles, equipment or other items or intentionally act in any way that could cause damage to the facility, or equipment or fixtures or persons within the facility. I agree to observe and obey all posted rules, and further agree to follow any oral instructions given by Pickleball Euphoria staff. When onsite for hourly court rentals or private events, I agree to limit facility usage to the hours secured per my rental agreement.  I understand and agree to comply with additional use policies as follows: Alcohol is not allowed on the premises. No food, outside  chairs, unattended children, non-Pickleball related athletic equipment  or items that might scratch or scar the court surface allowed inside the facility. Pickleball Euphoria is not responsible for lost or stolen property.  

ASSUMPTION OF RISKS AND RELEASE. I understand that there are certain inherent risks associated with the activity of Pickleball and I assume full responsibility for personal injury to myself and further release, hold harmless and discharge Pickleball Euphoria from and against any claim or cause of action arising out of injury, illness, disability, loss or damage arising out of my use or presence upon the facilities whether caused by the fault or negligence of myself, Pickleball Euphoria or other third parties including without limit bodily injury or death. As a condition of use of the facility and participation in any activity on the premises, all entrants agree to follow any and all policies, procedures and requirements established by Pickleball Euphoria or Resurrection Lutheran Church or by any federal, state or local government including without limit to ordinances, orders or laws applicable to any Covid-19 regulations or those regulations related to unforeseen circumstances or pandemic-related events.

RELEASE OF LIABILTY. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS PICKLEBALL EUPHORIA AND PE PRO SHOP, as well as their respective directors, officers, volunteers and/or employees, other participants, sponsors, and if applicable, owners and lessors of premises ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. 

BY BEING ON PREMISE AT PICKLEBALL EUPHORIA, WHETHER AS A PARTICIPANT, OBSERVER, VENDOR, CONTRACTED SERVICE PROVIDER OR VISITOR, I ASSUME ALL RISK AND AGREE TO THE THIS RELEASE OF LIABILITY AND HOLD HARMLESS AGREEMENT IN IT’S ENTIRETY.
